In order to engage in boss battles, players must create a Horror Potion using Weeping Obsidian. However, it is important not to drink or hold the potion for too long, as it will result in death. To transform a Skeleton, Zombie, or Enderman into a boss, the potion must be given to the chosen monster.

The first boss is Terror Man, with 450 health and a damage of 10. This boss will spit its own organs when standing still before attacking, dealing 20-30 damage. Players must then run for 30 seconds to avoid being hit, otherwise the countdown will restart. Upon defeat, the boss will drop organs and rotting flesh that can be used to create a cannon by adding Iron and Wood.

The second boss is a disfigured, primordial creature with 450 health, high speed, and a damage of 10. It can throw a sword that deals 20 damage and also summons Skeletons with helmets. This boss drops Disfigured Bones, which can be crafted into a Bone Blade to summon Skeletons.

The final boss is Terror, a primordial creature with 600 health that can teleport. It may randomly enter a Shield phase, where it hovers above the ground with a rune appearing beneath it. Players must break the rune to proceed. At 25% health, the boss enters a rage state, becoming stronger and faster. Upon defeat, it drops the Eye of Terror, which explodes and inflicts Exhaustion on nearby players.
